2010 Investment Schedule


Amusement; automotive; construction; finance companies; publishers & printers; radio & TV stations; restaurants, cafes & taverns; retailers; services; transportation; wholesale & distribution; diversified; hospitals and health care centers; and not-for-profit organizations:
$240.00 for 1-3 employees. Add $8.00 for each additional employee.
(2 part-time employees to be figured as 1 full-time)

Manufacturing and Processing:
$310.00 for 1 to 10 employees. Add $10.00 for each additional employee.

(2 part-time employees to be figured as 1 full-time)

**This category has a $3,100.00 investment cap.

Apartments/Manufactured Housing/Modular Home Parks:
$240.00 + $3.50 per unit
Nursing Homes:
$230.00 + $3.40 per room

Banks, Savings & Loans, and Credit Unions:

Less than $30 million in assets $240.00 + $18.50 per million dollars of total assets.
Over $30 million in assets $505.00 + $18.50 per million dollars of total assets.

Hotels & Motels without restaurant and/or lounge:

$237.00 + $3.50 per room

Hotels & Motels with associated restaurant and/or lounge unit:
$449.00 + $3.50 per room

Insurance & Real Estate:
$240.00 each for first 3 principals + $12.50 for each licensed employee.
Add $49.00 for each additional principal after first 3.

Professionals: Accountants, Attorneys, Dentists, Physicians, Surveyors:
$387.00 for first principal and $173.00 each for 2nd, 3rd and 4th principal; and $150.00 for each principal thereafter.

Utilities:
Negotiated

Colleges & Universities, Government Institutions and School Corporations:

$281.00

Membership Organizations:
$240.00 for 1 contact representative
Churches:
$100.00

Associate (non-voting member) Retired Persons, Persons Employed by Non-Member Firms, Civic or Social Clubs:
$120.00

* There is an initial $30.00 processing fee payable with membership application. There is a $5.00 service charge per billing for accounts other than annual.

** Dues for prospective members who do not fit into one of the above categories will be determined by the board of directors.
